The "5" in "5.1" denotes the five audio channels: left front, right front, center front, left rear and right rear. The ".1" is for a subwoofer and usually taps the left and right front channels but can also tap the center channel.
Your sound card has a bunch of output jacks that match up to these channels. The green jack is the left and right front channels. The grey jack is left and right rear. These are simple stereo jacks and you can use a 1/8" to L/R RCA separator to separate the signals. If you have a black jack then that's left and right side channels for a 7.1 configuration. It works just like the green and grey jacks. The orange jack is the center channel and is a mono jack. A 1/8" mono Y-splitter will split the signal for center channel and subwoofer.
